Non-Refundable Appointment Policy with Dr. Michael Spann

Appointment Fee-To ensure efficient scheduling and availability of care for all Michael Spann MD patients, the clinic maintains a non-refundable/non-transferable appointment policy. A $75 fee is collected at the time any cosmetic appointment is scheduled. regardless of attendance, cancellation, or rescheduling.

 

Cancellations & No-Shows-Cancellations, reschedules, or failure to attend an appointment (no-show) will notqualify for a refund. Missed appointments prevent other patients from receiving care and result in lost clinical time.

 

Late Arrivals-Patients arriving late may have their appointment shortened or forfeited at Dr. Spann’s discretion. Late arrivals that cannot be accommodated will be treated as a no-show and remain non-refundable.

 

Rescheduling (If Applicable)-If rescheduling an appointment with Dr. Spann is permitted, it must be requested at least 5 business days in advance. Rescheduling does not guarantee fee transfer unless explicitly approved by the practice.

 

Provider Cancellations-If the appointment is canceled by the Dr. Spann or the clinic, patients will be offered either: a full refund, or the option to reschedule at no additional cost.

 

Acknowledgment- By booking a cosmetic appointment with Dr. Spann, you acknowledge and agree to this non-refundable appointment policy.

 

Surgery Reservation Cancellation Policy

Reservation Fee Terms- To secure your cosmetic surgical date, a $500 reservation fee is required at the time of scheduling. The reservation fee is non-refundable/non-transferable. The fee cannot be transferred to another surgical date or procedure, it may not be applied to another patient or product, if the surgery is canceled or rescheduled.

 

Medical Clearance & Patient Responsibility- Patients are responsible for submitting all required preoperative evaluations, medical clearances, and laboratory testing 1 month before their surgical date. Failure to obtain medical clearance, evaluations, and labs does not exempt the patient from the reservation cancellation policy.

 

Cancellation or Rescheduling- If the patient cancels or reschedules the surgery for any reason, the reservation fee will be forfeited. A new reservation fee will be required to schedule a new surgery date.

Acknowledgment- By scheduling cosmetic surgery and paying the reservation fee, the patient acknowledges and agrees to the terms of this policy.
